Apply for a U.S. passport at City Hall
Layton City Hall is a U.S. passport acceptance facility designated by the U.S. Department of State. You apply in person, by appointment, and can have your passport photo taken on site.
Hours & Appointments
- Processing: Monday–Friday, 9:00 a.m.–3:30 p.m. (closed major holidays)
- By appointment only
- Applications must be completed before 11:00 a.m. for same-day processing
- Complete all forms in black ink before you arrive
Who Must Appear
- All applicants must appear in person
- Children 16–17: one parent must be present to sign
- Children under 16: both parents must be present to sign (see travel.state.gov for exceptions)
What to Bring
- Completed form DS-11 (do not sign until told)
- A color passport photo from the last 6 months (on-site photo service: $10)
- Proof of identity — driver license, state/military ID, or previous passport
- Proof of citizenship — certified birth certificate (not hospital-issued), naturalization/citizenship certificate, or prior U.S. passport
- Your Social Security number
- Payment (see fees below)
Fees
U.S. Department of State (pay by check or money order to “U.S. Department of State” — non-refundable):
- New book & card: $160 adult / $115 minor
- Book only: $130 / $100
- Card only: $30 / $15
- Expedited service: $60
- 1–2 day delivery (book): $22.05
- File search: $150
Layton City (card, cash, or check):
- Acceptance fee: $35 per application
- Photo (optional): $10
- 1–2 day application delivery (optional): $28.95
- 2% surcharge on credit-card payments over $75
